To immerse the patient in the situation, similar indications (as mentioned in Square Environment description) can be used.

After immersing the patient in the situation, the healthcare professional should ask them to elaborate on their thoughts. Again, we can find more suggestions in section 6 of this manual. 

In addition, you can practice handling attacks of panic by adding interoceptive effects and practicing relaxation or thought-stopping techniques.

Shopping in a large mall

Four environments in 360 format from the upper floors of the Mall that put the patient in different situations. In these environments, the patient will be able to move their head to look in different directions to experience a greater sense of presence than with the traditional environments.

  • “In the middle of the first floor” Located on the first floor in the middle of all the shops they will see people walking around and shopping.
  • “First floor, in front of the machinery” Another point of view between the shops on the 1st floor
  • “First floor, in front of the clothes” Another point of view between the shops on the first floor.
  • “On the tables of the second floor” In this video, in addition to seeing people walking through the Mall, the patient will also see city buildings in the distance and people eating at tables in restaurants.

Subway ride

Subway ride

This environment is tailored to patients who have a hard time taking underground public transportation. At the start of the simulation, the patient will be in a square standing in front of the underground entrance. 

SETTINGS VARIABLES
  • Gender
    Choose between a male or female body. A neutral body is selected by default.
  • Skin tone
    Choose the skin tone that best represents your patient to enhance the feeling of immersion.
  • Amount of people
    None / Few / Maximum
  • Clean environment
    Will expose the patient to a well maintained area. 
  • Dirty environment
    Their surroundings ( walls, floor, etc.) will appear more unattended for (stains, graffiti, etc). This will provoke a greater sense of anxiety.
  • Sanitary mask
    You can choose whether or not the people around you will have a mask.

Before the patient takes the train, the healthcare professional can elaborate on some of the patient’s thoughts about their surroundings or ask them questions while waiting at the platform entrance. The healthcare professional can choose different events to occur afterwards. 

EVENTS
  • Hall
    The avatar is located in the hall, where machines to buy train tickets are, before entering the underground station.
  • Tunnel
    The avatar enters the underground station and takes a moving walkway that leads to the platform through a tunnel.
  • Platform
    The avatar is on the platform, waiting for the train to arrive.
  • Metro arrival
    While on the platform, the avatar sees the subway arriving. He will be able to get on it and will start his trip in the carriage.
  • Next station
    As you travel inside the metro wagon, this event will simulate the avatar heading towards the next stop (the PA system will indicate that they are arriving at the next stop). 
  • Exit
    This event will cause the avatar to get off the wagon. When it has descended, the avatar watches from the platform as the subway leaves.
  • Breakdown On/Off
  • Breakdown On/Off
    Activating this event will cause the train to come to an unexpected halt inside the tunnel. You will hear a braking noise, the lights will turn off and the PA will announce that there has been a momentary breakdown that will soon be repaired. The train will remain stagnant until the healthcare professional allows the simulation to resume (clicking on the button Off). After that , the train will continue its journey to the destination. Keep in mind that this option will only be available when the patient is inside the train, otherwise it will be blocked.

Shopping in a small mall

Shopping in a small mall

Four environments in 360 format that propose that the patient goes all the way out of the subway to enter the Mall. The patient will also be able to observe all the people entering and exiting as they make their way to the first floor.

  • “Subway exit” The patient is next to a set of stairs on the ground floor of the mall observing people walking by and shopping from a certain distance. 
  • “Street. Far away from the mall entrance”  In this case the patient is standing outside on a street about 30m away from the shopping center. They can see all the people entering the center and walking around the area beside the city traffic.
  • “At the mall entrance” This environment, in 360 format, takes place inside of the mall near the entrance. The patient will see quite a few people shopping around and will be able to experience how it feels when it’s crowded. 
  • “In the middle of the first floor” Located on the first floor in the middle of all the shops surrounded by people walking around and shopping.

Four environments in 360 format that show different situations in a Mall with many people. In these environments, the patient will be able to move his head to look in different directions and experience a greater sense of presence than with the traditional environments.

  • “Ground floor on stairs” The patient is on the ground floor of the Mall next to the stairs observing people walking by and shopping from a certain distance.
  • “In the middle of the ground floor” In this scenario, the patient is in the middle of the ground floor surrounded by a multitude of people passing by from all sides.
  • “Ground floor next to a column” The patient is still surrounded by people on the ground floor, but this time, standing next to a column.
  • “Ground floor next to a column” This simulation is very similar to Event 3, but from another point of view on the ground floor.

On the way to shop

  • “Underground exit” This environment shows the exit of a subway stop. Two main moments can be distinguished: when a lot of people enter and leave the metro station (because a train has arrived) and another one in which there is not much fluidity of people since only people who are entering appear.
  • “Street away from the entrance.” In this case the patient is in the street outside a Mall about 30m from him, so you can see all the people who enter and walk through the area, as well as the traffic of the city
  • “Street near the entrance.” In this case the patient is also in the street but this time much closer to the entrance of the Mall.
  • “At the entrance to the Mall.” This environment shows from a 360º perspective the interior of a Mall right next to the entrance. The patient will be able to see different people strolling and shopping, as the site will be quite crowded.

Moreover, the healthcare professional therapist can control exposure to anxiogenic interoceptive sensations.

  • “Hall“ The boy is downstairs waiting for the elevator to arrive
  • “Boy goes to the elevator“ The boy takes the elevator and meanwhile, you can hear the voice of a girl who appears to be talking to the patient. 
  • “Girl says goodbye“ In this simulation, the girl that was talking to the patient is in a doorway and appears to be saying goodbye to the patient. After saying goodbye, she closes the door.

Moreover, the healthcare professional can control exposure to anxiogenic interoceptive sensations.
